1. Introduction

GOAL Physiotherapy and Sports Performance (“GOAL,” “we,” “us,” or “our”) respects your privacy and is committed to protecting the personal information you share with us. This Privacy Policy explains how we collect, use, disclose, and safeguard information when you visit our website, www.goaldrivenpt.com (the “Site”), fill out a form, book an appointment, or otherwise interact with us online.

This Policy applies to information collected through the Site. If you are a patient or client of GOAL, protected health information (“PHI”) created or received in connection with your clinical care is governed separately by our HIPAA Notice of Privacy Practices, which is provided to all patients and available upon request. Where there is a conflict between this Policy and our HIPAA Notice of Privacy Practices regarding PHI, the HIPAA Notice of Privacy Practices controls.

  • Respond to inquiries, schedule evaluations, and communicate with you about your request
  • Operate, maintain, and improve the Site and our services
  • Send appointment reminders, follow-ups, and service-related communications
  • Send marketing communications, such as newsletters, educational content, or event invitations, where you have opted in or as otherwise permitted by law
  • Analyze Site usage and marketing performance to improve our outreach and content
  • Comply with legal obligations and protect the rights, safety, and property of GOAL, our staff, and our clients

4. How We Share Your Information

We do not sell your personal information. We may share information with:

  • Service providers who perform functions on our behalf, such as website hosting, scheduling software, email/SMS platforms, and analytics providers, under agreements that limit their use of the information to providing services to us
  • Advertising platforms, in aggregated or hashed form, to measure ad performance or build lookalike audiences, where permitted by your cookie/marketing consent
  • Professional advisors, such as attorneys or accountants, where necessary
  • Legal or regulatory authorities, where required by law, subpoena, or court order, or to protect the safety of any person
  • A successor entity, in the event of a merger, acquisition, or sale of some or all of GOAL’s assets

Notwithstanding the above, mobile phone numbers and SMS opt-in data are never shared with third parties or affiliates for marketing purposes, as described in Section 5.

5. Text Messages and Email

Text Messages (SMS). If you provide your mobile number and opt in, GOAL Physiotherapy and Sports Performance may send you text messages. These may include appointment reminders and confirmations, scheduling updates, responses to your inquiries, account and billing notices, and, if you separately opt in, promotional or educational content about our programs and events. Message frequency varies. Message and data rates may apply. Consent to receive text messages is not a condition of purchasing any service.

Text messaging originator opt-in data and consent are excluded from all information sharing described in this Privacy Policy, including Section 4.

Email. If you provide your email address and opt in, we may send you appointment-related communications, follow-ups, and promotional or educational content. You may unsubscribe from marketing emails using the link in any message. Unsubscribing from marketing emails does not stop appointment-related or transactional emails needed to deliver care you have requested.

6. Cookies and Tracking Technologies

The Site uses cookies and similar technologies to recognize your browser, remember preferences, and analyze traffic. This may include tools such as Google Analytics and advertising pixels (e.g., Meta Pixel). These tools may collect information about your visits to this and other websites to help us understand Site performance and deliver relevant advertising.

You can control cookies through your browser settings. Disabling cookies may affect some functionality of the Site. Where required by law, we will present a cookie consent mechanism and honor your choices.

7. Data Retention

We retain personal information collected through the Site for as long as reasonably necessary to fulfill the purposes described in this Policy, maintain accurate business records, and comply with legal, tax, and regulatory obligations. Clinical records containing PHI are retained in accordance with Connecticut state law and our HIPAA policies, which generally require longer retention periods than general marketing data.

8. Your Choices and Rights

Depending on your location, you may have rights to access, correct, delete, or restrict the use of your personal information. To make a request, contact us using the information in Section 11. We will respond in accordance with applicable law. Requests related to PHI should be directed to us in accordance with the procedures described in our HIPAA Notice of Privacy Practices.

The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site without parental involvement. Where a minor’s information is submitted (for example, a youth athlete’s intake or a parent inquiring on a minor’s behalf), it is understood that a parent or guardian has provided or authorized that submission.

10. Data Security

We use reasonable administrative, technical, and physical safeguards designed to protect information collected through the Site. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
